When you come in for your initial visit please be sure to arrive early (at least 15 minutes in advance) in order to fill out your initial paperwork if you have not already done so. Also, please bring a list of all medications you are taking. If you need antibiotics prior to dental work, please take them as prescribed. If you are not sure if you need antibiotics, please contact your doctor prior to your visit.
Please note that we are a fee for service office and we don't participate with insurance companies. Nonetheless, we are happy to assist you in making your insurance claim.
Please understand that payment is expected at the time of service.
